INSTRUCTIONS: Please use this form to document the storage of expired chemicals in the Expired Chemical Cabinet prior to disposal. In the "Location" fields, please specify the lab or room where the chemical(s) originated. Provide as much information about the product(s) as possible. Once all containers have been entered, click the "Submit Pickup Request" button at the bottom of the page. Ensure that the expired chemical is placed in the correct bin based on its Hazard Classification. After submittal, you will be prompted to print a pickup request label. However, labels are not required for chemicals left in the storage cabinet. (Note: Ensure bins are not overcrowded and report any spills or leaks to EHS.)
