Chemical/Bio Waste Pick-up Request Form

If you have hazardous or regulated waste that needs to be collected for proper disposal, please complete the form below. Hazardous wastes include: Biohazardous/Medical Waste, Hazardous Waste (ignitable, reactive, toxic, corrosive), Regulated wastes include: batteries, pesticides, mercury containing devices, and fluorescent lamps and bulbs.


  1. In the spaces provided below enter your contact information. If you are logged into the system, your contact information will automatically appear.
  2. Add the location by typing the campus code (e.g., WC, NLV, HN, ALC, etc.) and select the corresponding building name. Then select the room number and provide any additional information on the specific location (e.g. “in fume hood”).
  3. Indicate the accumulation start date (typically today’s date).
  4. Indicate the type and description of the waste, including the quantity of containers or articles.
  5. If applicable, indicate the container’s contents and include the appropriate % for each chemical present in the waste, if applicable. If you’re uncertain, put an approximate %. If the container includes additional chemicals, select “Add Container Contents” and enter the appropriate information.
  6. Identify the specific hazards associated with the waste, and indicate whether the waste is solid, liquid, or gaseous, and pH, if applicable (except EHS Use Only).
  7. If you have additional containers or articles that need to be picked up, select “Add Another Container” at the bottom of the window and enter the appropriate information.
  8. If there is only one container or article for pickup, you can submit the request by selecting “Submit Pickup Request”.


Once a pickup request has been submitted, the requester will receive an email notification confirmation of their request for the pickup. Pick-ups are typically scheduled weekly, please allow a minimum of one week for waste pick-ups. If you have not had your waste picked-up within 10 business days, please contact EHS at 702-651-7445 or ehs@csn.edu.
